Pre-registration Required for Surgery
Prior to your child’s schedule procedure, you will need to pre-register. A representative from the Admission/Registration department will contact you prior to your child’s scheduled surgery to complete your telephone pre-registration. Following your pre-registration phone call, you will be required to come in and meet with a nurse in the pre-admit center. To shorten your wait time, we recommend you call ahead and make an appointment for this visit.

Please bring the following with you to your scheduled pre-op appointment:
- Picture I.D.
- Physician order/any paperwork from physician
- Insurance card
- Check, cash or charge for co-pays
- Guardianship papers, if you are not the biological parent
